Common and simplified norms for Investor Service Requests
SEBI vide Circular No. SEBI/HO/MIRSD/MIRSD/RTAMB/P/CIR/2021/655 dated 3rd November 2021, read with Circular dated 14th December 2021, had prescribed common and simplified norms for processing investor’s service requests and norms for furnishing of PAN, KYC details, Nomination, etc. for shares held in physical form.
It may be noted that SEBI has directed the Company/RTA to process all service requests received effective 01.01.2022, if and only if, they are submitted in the prescribed form along with the specified supporting documents. You may please click here for the Prescribed Forms.

As per SEBI (LODR) Amendment Regulations, 2022 dated January 24, 2022 read with Circular SEBI/HO/MIRSD/MIRSD_RTAMB/P/CIR/2022/8 dated January 25, 2022, the listed companies shall issue the securities in dematerialized form only on processing of Investor Service Requests viz, Issuance of duplicate securities certificate, Sub-division / Splitting of securities certificate, Consolidation of securities certificates/folios, Transmission, Transposition etc., The requests shall be submitted in Form ISR-4.
